1. Documents for ship entry procedures
The person carrying out the procedures submits and presents the following papers and documents:
– Documents to be submitted (originals), 01 copy of each type, including (sample documents specified in Appendix Decree 58/2017/ND-CP):
+ Submit to the Maritime Port Authority:
General declaration according to Form No. 42, List of crew members according to Form No. 47, List of passengers (if any) according to Form No. 48, Declaration of dangerous goods (if any) according to Form No. 49, Port departure permit;
+ Submit to Border Guard:
General declaration according to Form No. 42, List of crew members according to Form No. 47, List of passengers (if any) according to Form No. 48, Declaration of weapons and explosives (if any) according to Form No. 36, Declaration of persons hiding on board (if any) according to Form No. 34;
+ Submit to border customs:
General declaration according to Form No. 42, List of crew members according to Form No. 47, List of passengers (if any) according to Form No. 48, Cargo declaration (if any) according to Form No. 43, Declaration of information on bill of lading secondary goods (if any) according to Form No. 13, Dangerous Goods Declaration (if any) according to Form No. 49, Ship's reserve declaration according to Form No. 50, Declaration of taxable, prohibited or restricted luggage of passengers, crew members, and stowaways (if any) according to Form No. 51;
+ Submit to Medical Quarantine:
Maritime health declaration form according to Form No. 52, Medical declaration form, body, remains, ashes (if any) according to Form No. 33, Medical declaration form for medical microbiology samples, biological products , tissues, human body parts (if any) according to Form No. 14;
+ Submit to Plant Quarantine: Plant quarantine declaration (if any) according to Form No. 53;
+ Submit to Animal Quarantine: Animal quarantine declaration (if any) according to Form No. 54.
– Documents that must be presented (originals), include:
+ Submit to the Maritime Port Authority:
Certificate of ship registration, certificates of technical safety of ships according to regulations, professional certificates of seafarers, seafarers' books, Certificate of insurance or financial guarantee for civil liability for fuel oil pollution loss, Certificate of civil liability insurance of the shipowner for environmental pollution for ships transporting petroleum, petroleum products or other dangerous goods, Certificate Receive ship security according to regulations, Document approving foreign ships for specific activities (if any);
+ Present to border guards: Passport or valid international travel documents of crew members and passengers;
+ Medical Quarantine submission: International vaccination card of seafarers, Medical quarantine certificate, Certificate of exemption from ship cleaning/Certificate of ship cleaning (if any);
+ Plant Quarantine Submission: Plant Quarantine Certificate (if any);
+ Animal Quarantine Submission: Animal quarantine certificate of the exporting country (if any);
+ When the relevant specialized state management agency requests to present: Passport, international vaccination card of the passenger (if any).
2. Procedures for completing entry procedures for ships
Pursuant to Article 89 Decree 58/2017/ND-CP Regulations on ship entry procedures are as follows:
Step 1: The person carrying out the procedures shall notify and confirm the ship's arrival at the seaport according to Section 1 and Section 2
Step 2: Based on the notification and confirmation of the ship's arrival at the seaport, at least 02 hours before the ship arrives at the expected location at the seaport, the Director of the Maritime Port Authority, based on the actual conditions and technical specifications of the ship, type of cargo, port's dispatch plan, pilot organization's ship navigation plan and opinions of state management agencies to consider and decide to approve ships entering the port through the dispatch plan according to Form No. forty six Decree 58/2017/ND-CP.
In case the ship is not approved to enter the port, the person carrying out the procedures must be notified and clearly state the reason.
Step 3: No later than 02 hours from the time the ship has anchored at the wharf or 04 hours since the ship has anchored at other locations in the port waters according to the dispatch plan, the person carrying out the procedures shall submit and present the required documents. sheets and documents according to Section 3.1
Step 4: No later than 01 hour from the time the person carrying out the procedures has submitted and presented all documents as prescribed in Clause 3, Article 89. Decree 58/2017/ND-CP, state management agencies handle and resolve specialized procedures and immediately notify the Port Authority of the results and the Port Authority completes entry procedures for ships;
In case the ship has not completed the procedures, it must be notified and clearly state the reason.
